My nominee: F.A.S.T. Shampoo and Conditioner.
Used daily, its supposed to ramp up your growth rate to 1" a month. I used it 3-4x a week for a little over a month last spring. If you have kinky curly hair longer than oh I dunno...3", DON'T USE THIS STUFF! It has the dreaded SLS in it. What was I thinking? 
It turned a section of my hair into a locked matted mess. I managed to separate my hair with minimal hair loss.

What I can't get over is her little *DOUBLE DUCT TAPING INCIDENT*. First off who in the history of hell DUCT TAPES labels after being in business for 2 years 
(@Kurlee, she pointed out the specific type of said tape 
...had me rollin'). However not only did she duct tape my label, but she sent me the wrong product with the label of the product I wanted DUCT TAPED on top of the actual DUCT TAPED label ....
