Where are my Profectiv samples?

Has anyone else filled out the questionnaire on their website and never received the samples? I had to have done this almost two months ago and I still never received anything. I got my Mizani Thermastrength sample in a much shorter time period. Maybe I should re-fill the questionnaire out?

It's been 2-3 months since I filled out the questionnaire and I never got mine, either. I thought they ran out of samples but I noticed that the offer is still on the site...
