My definition of truly moisturized hair is hair that feels and looks moisturized even when you don't apply any products to it.

I think your hair is truly moisturized when it feels and looks healthy and shiny, and is not beaking. Sometimes my hair will feel and look healthy but then at the same time its breaking. I then rinse and add additional moisture and the breakage stops instantly.
