Well, I think if the hair is long enough, you can have a beautiful bun wth no fillers. Even if it is not very long, you could tease it for fullness, but I know no one here would do that, right 
? Because my hair is not long enough for the beautiful buns sans fillers, I use one of these
as a ponytail holder and wrap the hair around that. It's still a filler, just not a doughnut. I am thinking that's probably not the answer you wanted, but it's the only one I've got
. I am anxious to see more responses though, because I'd like to find a way without pieces if it's possible with medium hair

Lorainne aka LorraineG has a beautiful unaided bun in her album. I do my bun this way very often and it comes out nice everytime.

I don't wearing my real hair in a bun, however, I do wear the straight phony pony's in a bun. Normally I just take the hair and start twisting it into a bun as I would if my hair was that length. Each section of hair I twist and wrap around I secure it with bobby pins. As soon as I know it, I have a really pretty bun. I plan to take pictures and put it in my album soon.

My buns contain no filler but they are hair pinned buns. My hair is not long enough to twist and have a decently sized bun. I air dry my hair when I plan to wear one because it leaves my hair VERY full. Here's what I do:
1. Load up the moisturizer.
2. Put on the scrunchie.
3. Make 4 sections (front, side, side, back)
4. Roll a section at a time on my fingers and secure with a very large hair pin.
4. Repeat until all sections are done
5. Use fingers to smooth/arrange the sections so you cannot see any separation.
I have a pic in my album under "Up Dos."

I've missed LHCF!
I use the same Revlon hair scrunchie that Dreemsold showed in her post above. It's available at Sally Beauty (under $10) and if you get a close color match, its practically undetectable. You will need at least 2 "ouchless" hair elastics for this and bobby pins if your hair is not quite long enough yet.
1. Lightly brush and smooth hair to make a ponytail with the first elastic (a little serum or leave in conditioner is nice, and if you usually moisturize your ends, go right ahead)
2. Slide the hair scrunchie over your pony, looping again as needed.
3. Fan/Separate your hair around the hair scrunchie and use the second elastic to secure and tuck all the ends under and around the new bun. Depending on how long your real hair is, this bun circle may look very concentric or a little off which actually makes it look more real.
4. Secure with another elastic if you'd like
5. In the evening when I take this bun down, I like to take a minute to brush or comb the hair on the scrunchie to detangle it, so its ready to roll the next morning.
Alternatively, if your hair is long enough you could try a Scunci MagicBun clip - avail. at Walgreens, Walmart, Kmart etc.

lol...I need to try the one with the hair....they have some of those at walgreens for a few bucks.
Every time I think of a bun it makes me laugh bc my friends at school make fun of me wearing one a lot..they say I Look like Florence Johnston. I agree I look better with my hair styled other ways bc I have a big head....but u gotta do what u gotta do.
A few other styles that compliment me a little more but are still protective are a french braid, a loose twist with some hair left out, the top part in a bun with the bottom part out.....but my hair looks much better if I roller set or wear a twist or braid out.
any suggestions for making a bun look better on a big *** head???????? LOL

This is what works for my hair:
1. place hair in a ponytail (sometimes I don't use a ponytail holder and sometimes I do)
2. section ponytail in two and do a two strand twist
3. coil the two strand twist around and around
4. secure with Good Hair Days Pins or with a cute pair of hair sticks

When I don't use a filler I gather my hair up into a ponytail that is earlength or higher.
1. I take my hair and do a thick twist.
2. I use 4 hairpins and after I've wrapped my hair to make a donut I pin the hair.
You can also section your hair into two and do either a twist out like Allandra said, or two braids (I do this one). I then follow Allandras method and secure using hairpins.
