How Do You Prevent "Dead Spaces" When Making Two Strand Twists?

What I mean by "dead spaces" are large gaps in between some twist. Like you will have twists side by side, but in between many twists is like gaps where it's basically just scalp? Could it be some people just do not have thick enough hair or could it be the way you divide the hair when styling?
